Case Management

About The Program

CMHA Toronto’s Case Management program works with individuals 16 years of age and older who have been diagnosed with a serious mental illness and who require support to live and work in the community. These services assist individuals in meeting their basic needs (e.g., food, shelter, finances) and encourage the development of skills needed to live independently and improve quality of life.

Clients work with case managers, who promote independence and provide ongoing support as needed in order for the client to stabilize, achieve their goals and improve the quality of their life. Case managers also work to coordinate required services across the mental health system, as well as other services systems (e.g. criminal justice and developmental and addiction services). Integral to this process is the direct involvement of the client and the development of a caring, supportive relationship between the client and the case manager, and where appropriate, with the client’s family and supports.

How IT Works

  1. The client applies through The Access Point, an intake service that provides coordinated access for Case Management services in Toronto.
  2. Once accepted, the client is matched with a case manager.
  3. The case manager then meets with the client and together they identify goals, create a plan and work together to advance recovery.

Who are Case Management services for?

Case Management services are for individuals 16 years of age and older who have been diagnosed with a serious mental illness. 

Where are Case Management services offered?

Case Management services are offered in clients’ homes, the community and in CHMA Toronto offices.

What areas does Case Management serve?

CMHA Toronto operates separate Case Management programs out of our East and West sites. These programs serve individuals in Toronto, North York, Etobicoke and Scarborough. 

What languages are offered?

Services are offered in Somali, Tamil, Dari and Pashto in Scarborough, and in French and Italian west of Yonge Street 

How do you apply for Case Management services?

All applications to CHMA Toronto’s Case Management services are through The Access Point, an intake service that provides coordinated access for Case Management services in Toronto.

Referrals are typically made through professionals, but self-referrals are also accepted. An application form can be obtained by calling The Access Point at 1-888-640-1934 or by visiting their website at www.theaccesspoint.ca.
